Case Manager Job Purpose:
Oversee the daily case management of inpatient residents.
Comply with all state, regulatory and Freeman Recovery Center policy and procedures for quality patient care, confidentiality and privacy.
Primary
Responsibilities:
Discharge planning, referrals, scheduling appointments, patient documents Work with local and statewide providers to develop a strong working relationship to ensure the utmost care for residents Maintain up to date, complete, and accurate patient records Communicate with residents regularly regarding case management and discharge planning Coordinate care with other community providers as needed Excellent organizational, and communication skills Knowledgeable in 42/CFR Part 2 and HIPAA Experience in Behavioral Health preferred Education and experience, degree and/or certification required (if applicable):

Bachelor's Degree human services or behavioral science preferred 2.
Certified Case Manager 3.
Organization and time management skills 4.
Experience in Behavioral Health Preferred 5.
CPRS Certified Preferred Working Relationships:
As a representative of Freeman Recovery Center, LLC, all comments, actions and behaviors have a direct effect on Freeman Recovery Center, LLC and its image and perception of quality care for the individuals we serve.
Interactions with patient(s), staff, referral sources, guests and visitors, volunteer(s), and supervisors must be in a manner that is friendly, supportive, courteous, respectful and professional.
