Participate in departmental operations through analysis of Case Management practices and outcomes. Monitors compliance with third party payor requirements, government regulations and JCAHO standards to support quality of care for patients and maximize reimbursement. Daily monitoring of appropriateness of transfers, admission status, and length of stay, and the appropriateness and timeliness of post-discharge/departure symptom management through access of community resources. Facilitates interdisciplinary team approach to meeting the broad spectrum of the patient???s healthcare needs. Appropriate interpersonal and communication skills. Evidence of analytical and decision-making skills. Participates in Performance Improvement activities to assure continuous Quality Improvement. Maintains cultural and diversity awareness when dealing with patients, visitors, co-workers and other customers. Promotes a supportive, team-work oriented environment. Current Tennessee RN licensure. Three years of Case Management and/or Utilization Review/Discharge planning experience preferred. Computer application skills preferred. Basic Healthcare Provider (BLS) Certification required within 90 days of hire or promotion.
